What to Bring to your First Appointment
To provide our patients with the best treatment, we request you bring the following items to your initial visit to our practice:
- Insurance Information / Card
- Any medical records pertaining to your visit (list of medications you are taking, pre-existing conditions, etc.)
- Photo Identification
Please arrive at least 15 minutes before the start of your scheduled appointment time to fill out necessary paperwork.
